Please know that any appointment you make requires a 24 hour notice for rescheduling or cancellation (except for 15 minute Acute Visits). A $50.00 late cancel/rescheduling fee may be billed/charged to your credit/debit card if the appointment is cancelled/rescheduled in under 24 hours. If your appointment is in less than 24 hours and you need to make a change, please contact Healthy Roots Clinic directly.

Healthy Roots Clinic does not bill insurance directly but is happy to provide you with a superbill you can submit to your insurance for reimbursement.
